Set up an appointment in three easy steps

  1. Call me at 323.422.9433 first to determine if I am a good fit as a therapist for your needs.
    • After we’ve spoken on the phone, email me at michaeltfatula@gmail.com from your personal confidential email address (not from a work email address).
    • In the subject line write: “client request for insurance verification.”
    • In the body of your email, write your full name, date of birth, area code & telephone#.  Then write “I give Mike Fatula and staff permission to verify my  insurance benefit.”
    • In an attachment, put a close-up picture of the front and back of your insurance card.

  1. After I call you with the verification of your insurance, I will email the intake packet. Print it out single-sided, fill it out, and bring it in to your first appointment (for telehealth zoom session clients, email the completed intake packet to me). For in-person, in-office clients: if you’d prefer not to fill out an intake packet ahead of time, or you do not have access to a printer, you can come in half an hour before your appointment time to fill out an intake packet that will be waiting for you on a clipboard in the waiting room.

  2. For telehealth counseling clients only… Please download the one-page release form, sign, and email it to us at michaeltfatula@gmail.com.  (Since the beginning of the pandemic, I haven’t been conducting in-person, in-office sessions. I’ve only conducted telehealth sessions on a HIPAA-compliant platform of zoom.)
